Every guest works with staff to develop an individualized plan built around personal goals and coordinated support. Community partners provide many of these services on-site, creating a single place where people can access the resources they need to move toward lasting stability.
Access to healthcare, behavioral health, recovery support, and wellness services helps guests address immediate needs while building long-term well-being.
Guests are invited to explore faith, purpose, and personal growth through optional spiritual support provided by local congregations and community partners.
Life skills, financial education, employment preparation, transportation, identification documents, and other practical supports help guests prepare for independent living.
Lasting stability grows through healthy relationships. Guests build connections with staff, volunteers, mentors, employers, service providers, and one another as they move forward.
Case managers work alongside guests to identify housing opportunities, strengthen applications, connect with landlords, and prepare for a successful transition into permanent housing.
Criteria

Bridge Housing serves adults experiencing homelessness who have established ties to the Pottstown community and are committed to working toward greater stability through individualized goal planning and ongoing participation.
Prospective guests complete an intake process that includes eligibility verification, safety screening, and a conversation with a case manager to develop an individualized plan based on their strengths, goals, and immediate needs.
If someone is not eligible for Bridge Housing, our staff will do everything possible to connect them with another appropriate shelter, treatment program, or community resource.
Bridge Housing is designed to provide a safe and stable environment for all guests. Individuals needing detoxification or a higher level of care will be connected with appropriate treatment providers before entering the program.

Every guest's journey is different. Length of stay is based on individual goals and progress toward stable housing rather than a fixed timeline. Our goal is to provide the time and support needed for each person to move forward successfully, usually between six and nine months.
Guests have access to individualized case management and coordinated support from trusted community partners. Depending on their needs, services may include healthcare, behavioral health, employment assistance, financial coaching, legal support, housing navigation, recovery resources, and other programs designed to promote long-term stability.
Bridge Housing is a temporary program that helps individuals prepare for permanent housing. Staff work alongside each guest to identify housing opportunities, strengthen applications, connect with landlords, and build the skills needed for long-term success.
Admissions are based on program availability and completion of the intake process. Our team works with each prospective guest to determine eligibility, complete assessments, and develop an individualized plan before entry into the program.
